- Individual and joint interviews with a social
worker
- Completion of written self-assessments and
other assignments
- Contacts with your references by a social
worker
- Completion of physical examination form,
including sterility findings when applicable
- Counseling regarding infertility programs
when sterility is questionable
- Financial statement, credit check
- Criminal record check; civil record check for
domestic violence;
- Agency reserves the right to decline the
applicant(s) request for adoption at any
stage of the adoptive preplacement
assessment process. Also, the
applicant(s) may voluntarily withdraw at
any stage of the preplacement assessment
process.
